Thank you for participating in this year's High School Math Contest. We hope you enjoyed the contest!

Please follow the score report instructions carefully.

Instructions

1. Retain test papers of all students with a perfect score.
2. You must fill out all the required fields for us to accept this form. You will need a valid email address in order to use this form. A copy of the scores you submit will be emailed back to you as confirmation of your entry. You do not have to use the mail-in form if you submit your school's scores with this form.
3. For each student listed, use the checkbox for each problem to indicate whether the student's answer was marked correct or incorrect.
4. Results must be submitted no later than one week after the officially scheduled contest date.
5. Only teams of students from the official, accredited school of record are eligible for school awards.
6. You must score in accordance with the answer key. Appeals must be in writing. Mail appeals separately, or e-mail them to Steve Conrad <steve@mathleague.com>.
7. Optionally, you may use this form to submit names of additional high scoring students.
